Analysis of Tools for Diagnosing Autism Spectrum Disorder in the Indian Context-Juniper Publishers

Image
Juniper Publishers - Journal of Pediatrics Abstract Autism Spectrum Disorder (ASD) is a neurodevelopmental condition with varied manifestations and poses a diagnostic challenge. The prevalence of ASD has increased in a highly populated and demographically ‘young’ country like India. However, prevalence rates are varied due to differences in measurement methods. Moreover, caregivers tend to delay reporting for developmental concerns as compared to physical health problems. The current analysis focuses on two common diagnostic tools used in India for ASD i.e. the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders and the Indian Scale for Assessment of Autism; and compares these tools with the revised Autism Diagnostic Interview. The analysis describes strengths and limitations of each of these tools and provides recommendations for their use in outpatient clinical practice. Keywords: Autism Spectrum Disorder; Diagnosis; Analysis; India, DSM-5

Risk Factors, Diagnosis, and Current Practices in the Management of Intraventricular Hemorrhage in Preterm Infants: A Review-Juniper Publishers

Juniper Publishers - Journal of Pediatrics Abstract Intraventricular hemorrhage (IVH) is one of the major complications in premature infants, and its management is quite challenging. This review article describes the pathophysiology, risk factors, and complications of IVH. Further, it explains the current diagnosis and treatment of IVH, along with the recent advances in its management. IVH can occur due to fragility of germinal matrix vasculature, impairment of cerebral auto-regulation, coagulation disorders, genetic factors, and oxygen toxicity. Cranial ultrasonography has been the gold standard for screening in newborns for detecting abnormalities like IVH. However, the need of the hour is diagnostic techniques which not only provide information on brain anatomy, but also related to the hemodynamics. Thus, various techniques are being explored like near infra-red spectroscopy (NIRS), advanced magnetic resonance imaging (MRI) techniques, along with the use of biom

Congenital-Infantile Fibro sarcoma of Hand – A Case Report-Juniper Publishers

Image
Juniper Publishers - Journal of Pediatrics Abstract Congenital infantile fibro sarcoma is a rare malignant tumor of the soft tissue in the infant primarily developed in the soft tissue of distal extremities and occasionally in unusual location such as the lungs and retro peritoneum. It occurs mainly in children below the age of 5 years. About 250 cases have been reported in the literature so far, very few of them in newborns. Its clinical course differs from conventional fibro sarcoma, the prognosis is relatively good compared to adult forms and the 5 year survival rate can be more than 80%. We report a case of congenital infantile fibro sarcoma of the hand treated successfully by below elbow amputation. The child is now 5 years old with no recurrence. Keywords: Fibro Sarcoma; Congenital; Infantile Introduction Congenital infantile fibro sarcoma is an uncommon soft tissue malignancy. It usually affects the extremities [1]. Few cases involving the hand we

Atypical Presentation of Choledochal Cyst Type IV-A in a Hispanic Girl: A case report-Juniper Publishers

Image
Juniper Publishers - Journal of Pediatrics Abstract A type IV-A choledochal cyst is the second most common type of this rare congenital entity that is characterized by multiple dilatations of the intrahepatic and extrahepatic biliary tract. The most common clinical findings of this entity is abdominal pain, palpable right upper quadrant mass and jaundice. However, there are other clinical presentations that differ from the classical presentation that can delay the adequate management of the choledochal cyst. The optimal treatment is total surgical excision along with a hepaticoduodenostomy or hepaticojejunostomy. We present a case of a 22 month old female with an unusual presentation of a choledochal cyst. Introduction Choledochal cysts are rare congenital entities characterized by single or multiple dilatations of the intra and/or extrahepatic biliary tree [1]. Its incidence in Asian population is 1:1000, but in western populations is 1:100000 to 1:15000

Magnesium Sulfate for Fetal Neuroprotection: Correlation between Maternal and Infant Serum Magnesium Concentrations-Juniper Publishers

Image
Juniper Publishers - Journal of Pediatrics Abstract Background: Pregnant women at risk for preterm labor are commonly treated with magnesium sulfate (MgSO4) for fetal neuroprotection. Randomized controlled studies have shown that this treatment is associated with decreased risk of cerebral palsy. Nevertheless, the optimal MgSO4 dose for neuro protection has not been determined. Moreover, some studies suggest a potential toxic effect associated with high MgSO4 treatment dose and that elevated serum infant Mg concentrations (iMgC) may also be associated with an increased risk for various morbidities and mortality. Objective: To determine the correlation between maternal and infant serum Mg concentration among preterm infants exposed to MgSO4 for neuroprotection. Finding a correlation may contribute to decision-making regarding continuation or discontinuation of maternal treatment prior to delivery. Study Design: A total of 60 infants born at <32 weeks gesta
