Femoral Fractures in Children Under 2 years of Age in Western Australia: How Common is Inflicted Injury and What are the Risk Factors? | Juniper Publishers

  Abstract: Background: Femoral fractures in infants under 2 years of age are a common presentation to Australian hospitals. Studies from North America and Europe have reported inflicted injury rates between 10-60% in these children 4,6,7, however there have been no similar studies in Australia. The aim of this study was to establish the prevalence of inflicted femoral fractures in the West Australian population and to determine the risk factors for inflicted injury. Methods: A retrospective cohort study was performed on children under 2 years of age presenting to Princess Margaret Hospital in Western Australia with femoral fracture between 1998-2013. Cohorts of 228 patients with 235 femoral fractures were studied. The primary outcome variable was the percentage of cases determined to be inflicted injuries.   Abstract: This case report is about a 36 year old primigravida patient in Southern California. She worked a sedentary job, was severely overweight prior to becoming pregnant and was unable to carry the baby to term. This paper reports on all the problems of the patient and the pregnancy, Caesarean Section Delivery of the infant at 24 weeks and the subsequent death of the child at the age of 6 days. Written permission and all of the information was freely given to the authors by the father, the mother refused to talk about the mistakes she had made, her guilty feelings and the fact that she was ultimately the cause of the death of the child due to her obesity precipitated this refusal. Abstract: First and foremost, pregnancy is not a disease; it is a “state of being”. All mammals, humans included, are on this planet for one purpose and one purpose only, to procreate for the purpose of the continuance of the species. No religious arguments will be discussed in this paper as they are all irrelevant to the process of a normal human pregnancy. This paper is being written so it can be used as a quick review for physicians that may not do Obstetrics as their primary speciality. Many countries allow General Practitioners and Family Practitioners to manage pregnancies and deliver babies, as long as they have privileges to do this in the hospital. Usually however, non OB/GYN’s will not be allowed to be the primary surgeon on a Caesarean Section due to lack of practice and hospital rules, but they will be allowed to be First Assistant. A Monoamniotic Pregnancy Discordant for Limb Body Wall Complex and Complicated by Acute TTTS by Pace Carlotta in Juniper Publishers in Academic Journal of Pediatrics & Neonatology Abstract: We are reporting a case of monoamniotic twin pregnancy discordant for Limb Body Wall Complex (LBWC), complicated by acute twin-to-twin transfusion syndrome. The twins were delivered by an emergency cesarean section at 29+6 weeks ’ gestation because of persistent fetal tachycardia of the unaffected twin. The phenotipically normal twin was anemic at birth (donor twin), while postmortem examination revealed a condition of overloaded fetal circulation of the LBWC twin (recipient twin). This is, to our knowledge, the first case of acute TTTS in a monoamniotic pregnancy reported in Literature. In monoamniotic pregnancies discordant for LBWC careful fetal monitoring should be granted, as the presence of the malformation masks the classic signs of twin-to-twin transfusion syndrome ....
